Custom forms let you collect more than name and email. Add the questions that matter to your funnel: budget, timeline, project type, whatever. Each form gets its own URL and can also be attached to a card so it replaces the default contact-back form.
Custom forms are available on the Plus plan (1 form), Team plan (3 forms), and Pro plan (10 forms).
Creating a form
- Sign in and open Settings → Forms.
- Click New Form.
- Enter a name and a slug (this becomes part of the public URL:
businesscards.io/f/your-slug).
- Add a title and description that visitors will see at the top of the form.
- Customize the submit button text (default is "Submit").
- Save the form skeleton.
Adding fields
Open the form and click Add Field. For each field, pick:
- Type: text, email, phone, date, datetime, number, URL, textarea, or select (dropdown).
- Label: what the visitor sees above the input.
- Description: optional helper text below the input.
- Required: whether visitors can submit without filling this field.
Drag fields to reorder them. For select fields, add the dropdown options one per line.
Sharing or attaching the form
There are two ways to use a form:
- As a standalone URL. Share
businesscards.io/f/your-slug directly via email or social. Anyone with the link can submit.
- Attached to a card. Open a card in the editor and pick the form in the contact-form section. The card's "Send me your info" button now opens your custom form instead of the default one.
Where submissions go
Every submission lands in the form's entries page (Settings → Forms → [your form] → Entries) and is also recorded as a contact in your Contacts dashboard. You can export entries to CSV at any time.
CRM and Zapier
If you have HubSpot, Salesforce, or a Zapier integration set up, custom-form submissions flow there just like default contacts do. See the HubSpot, Salesforce, and Zapier articles for setup.
Duplicating a form
If you've designed a form that works well, you can duplicate it as the starting point for a new one. Open the form, click the menu, and pick Duplicate. The new copy gets a fresh slug; rename and edit as needed.
